Procedure
The registration form should be used for the Congress Registration, Hotel Reservation, Social Programme and Excursions.
All participants must submit a completed registration form. The ISTH Congress requires the full data for each individual participant, including updated postal address and Email address. Incomplete registration forms or registrations not accompanied by the completed registration form cannot be processed.
All registrations must be made using the official registration form or preferably through the online registration form.
Participants who have already registered and not yet booked their hotel should use the Hotel reservation direct link to do so.

* For postgraduates and students, a letter of confirmation from the Head of Department must be sent with the registration form.
The exchange rate at time of printing (June 2006) is CHF 1.- = E 0.65 / CHF 1.- = US$ 0.85
Early registrations will be accepted until 28 February 2007. The reduced fee will only apply when early registration is accompanied by payment.

Payment
Full payment should be received before the relevant deadline. To facilitate processing, participants should clearly indicate in all remittances their name, address and the purpose for which the payment is made.
Either of the following means can be used for payment:

Group Registrations
For group registrations, a minimum of 10 participants is required. Please contact the Congress Secretariat, MCI
Suisse SA, to obtain the group registration form.
Congress documents and bags can be collected as early pick-up upon request.
Confirmation
A confirmation of registration and payment status will be sent to the individual or company that requests registration. Confirmation will be sent only when the corresponding payment is received. Participants should bring the letter of confirmation to the registration desk at the Congress venue when collecting their documents.

Hotel Accommodation
MCI Suisse SA has reserved a large number of rooms in different hotel categories and proposes discounted room rates for ISTH 2007 participants. Only reservations made through MCI Suisse SA will benefit from these rates. The majority of the hotels are located in the city centre. The Congress Centre can be easily reached from all hotels by public transportation. Please make your reservation on-line or with the attached form to:

Since Geneva attracts many tourists, please reserve your room not later than 7 May 2007. Hotel accommodation is subject to availability, requests received after this date will not be guaranteed.
Reservations are made on a first come, first served basis. Availability of rooms in each category is limited.
Names of persons sharing rooms must be indicated. Rates are in Swiss Francs (CHF) and are per room, per night, service and VAT included.
Participants are asked to complete the Registration and accommodation Form indicating their choice of hotel by order of preference.
Reservation and Payments
Reservation and payment: Only guaranteed reservations will be accepted. To guarantee your reservation a deposit covering one night is required, either by bank transfer or by credit card (deposits will be deducted from the final hotel bill when paid by the participant at the hotel). Please indicate the participant’s name and “ISTH 2007” on ALL payments.
From 23 May 2007, only payments made by credit card will be accepted.
Inquiries
Any inquiries or requests for additional information, changes or cancellations of room reservations should be addressed directly to MCI by Email or in writing.
Group Reservations
Group reservations will be handled by MCI Suisse SA with separate contracts. Please contact MCI for further information.
Changes of Hotel Booking
All changes to hotel reservations will be performed by MCI. Please do not contact the hotel directly. All changes must be requested in writing. A handling fee of CHF 50.- per hotel room will be charged for every hotel modification received after 6 June 2007.
Cancellation of Registration, Social Programme and Accommodation
All cancellations must be sent to MCI Suisse SA in writing (fax, letter or Email).
For cancellations received before 23 May 2007, deposits will be refunded less CHF 100.- for administrative costs. After this date no refund will be possible.
In case of late cancellations (after 23 May 2007), rooms cancelled, cancelled nights or no-shows, we reserve the right to charge the full stay if the hotel room cannot be re-sold.
A handling fee of CHF 50.- per hotel room will be charged for every hotel modification received after 6 June 2007.
Cancellation of Excursions
Notification of cancellation must be made in writing. Full refund will be granted if written notice is received prior to 6 June 2007. For cancellation after that date, no refund will be made. Full refunds will be made if an activity is cancelled due to lack of participation.
Check-in/Check-out Time
Hotel check-in time is 15:00 or later. If you are arriving after 18:00, please notify in advance. Check-out time is noon.

MCI Suisse SA as the Congress planner, ISTH and ISTH 2007 committees claim no liability for the act of any supplier to this meeting, nor liability for: personal injury, the safety of any attendee while in transit to or from this event, for any loss or damage, for delays in transport by air, sea, rail, road, weather, in case of strikes, sickness, war or other causes.
Methods of Payment
Please see payment methods on page 17 for more information. Payment by personal checks cannot be accepted.
All payments have to be effected in Swiss Francs (CHF). For hotel bookings: a deposit payment for the first night must accompany the hotel accommodation form. Additional expenses such as mini-bar, laundry, etc. must be paid directly to the hotel when checking out.
